Saving tool bar icons

at whit's end here, when I add an icon to my tool bar it seems it doesn't want to stay. 

When I reopen or open a new model the icons are gone, how do I save them so they show up in the models old or new. I have no problem putting them in the tool bar but then gone when reopen. 

Any help would be appreciated
